  • Gestational diabetes and diabetes in pregnancy care, including shared care

  • Pre-conception care and family planning support for women living with diabetes

  • Prediabetes and diabetes prevention

  • Diabetes remission assessment and support, where appropriate

  • Complication screening and risk reduction

  • Group education and peer support services
