Newly diagnosed and ongoing diabetes care and education
A new diagnosis can bring a lot of questions, and it's normal if you are feeling overwhelmed at first. You don't need to understand everything right away — we'll take things at your pace, and help you build the knowledge and confidence to manage your diabetes over time.
Regardless of how long you have been managing your diabetes, we know the value of having reliable and trusted support. We’ll get to know you, keep you up to date on the latest management practices - even send you reminders or help you with making sure you have your supplies regularly! If you’re living with diabetes, we’re here to help.
Our approach
Current best-practice guidelines recommend that everyone diagnosed with diabetes receive structured diabetes education — not just once, but at key points along the way, including at diagnosis, annually, and whenever your circumstances or needs change.
At Health Seeds, transparent and evidence-based care is our priority - so you understand what’s going on with your health and health care. We follow best practice guidelines to help shape your care. From there, we work with you to build a plan that fits — matching you with the right practitioner, appointment format, and setting (including our group services, which combine structured education with peer support at a lower cost than individual appointments) so your care is both high quality and as accessible and affordable as possible.
How we can help
Every diagnosis is a little different, so we'll tailor your first appointment to you. Together, looking after your diabetes often includes:
Understanding your specific type of diabetes and what it means for you
Practical guidance on monitoring, medication, or insulin, where relevant
Building a day-to-day management plan that fits your life
Ongoing support as your confidence and needs change
Working alongside your GP and other healthcare providers, so your care stays connected
Monitoring and reducing your risk of complications over time, including relevant blood test screening
